The Emergency Assistance Fund was established in 1990 to provide emergency assistance for patients with Alzheimer's Disease or a related disorder and their family members.
Possible uses of the fund include, but are not limited to the following:
- transportation to medical appointments or diagnostic workups
- small equipment purchase or short-term rental of medical equipment
- funding of emergency respite service in the event of sudden caregiver illness (this includes the following: a licensed home care agency, a licensed adult day care center or a licensed residential facility)
- purchase of medicines
| Fund Guidelines |
- Applications should be submitted by a family member or health care professional.
- Applicant must live within the Northern Virginia Chapter's territory.
- The funds will be paid directly to the service provider.
- Pre-existing bills will not be considered.
- Funds are provided on a one-time only basis with a maximum of $500.
- Verification of Alzheimer's disease or a related dementia is required.
- Applications are approved by the executive director and/or designated staff.
- Extenuating circumstances will be considered on a case by case basis.
